Registration Information

Registration Information - Spanish

All forms may be downloaded from our website, the registrar's office will provide printed forms upon request. When printed forms are complete you may choose from the following options:

  • Scan and email the Application & Required Documents to the Registrar at: corina.nuby@mcdadeisd.net

  • Hand Deliver the Application & Required Documents to Central Office: 156 Marlin Street, McDade, Texas

  • Mail the Application & Required Documents to: Corina Nuby, Registrar P. O. Box 400 McDade, Texas 78650

Do not complete the New Student Registration forms if your child is a returning student or if you have completed a transfer or pre-kindergarten application that has not yet been approved by the district.

Registration Documents:

  • Certified Birth Certificate of Student

  • Copy of Student Social Security Card

  • Current Immunization Record

  • Copy of Parent/Guardian Current ID

  • Proof of Residency (i.e., utility bill, lease agreement, or tax statement)

  • Copy of Final Report Card (2024-2025 School Year) for Students in Grades 1st-12th
